District Six Medical Examiner - Records Request
Autopsy Reports:
Autopsy reports are provided to the next of kin without charge. This includes the spouse, parents, children, siblings, grandparents and grandchildren. If the autopsy report can be received by email, we will email requested reports as our preferred option.
Local hospitals, physicians and media can also receive the autopsy reports by using our contact form or by local fax number (727) 582-6820 at no charge.
Insurance companies, attorneys, and other businesses requesting an autopsy report can receive the report via email or local fax at no charge. If requesting the autopsy report be mailed, the requesting party shall supply our Records Custodian with a self addressed; stamped envelope to cover postage costs.
Other Public Record Requests:
For public record requests involving more than just the autopsy report, a fee of $0.15 per page will be charged. This fee is established pursuant to s. 119.07(4)(a)1 F.S. Pre-payment of the total amount of such requests is required.
If a self addressed stamped envelope with the appropriate postage is not provided, a $1.00 fee will be charged for postage and handling.
Other requests should be directed to our Records Custodian. This would include special requests by attorneys, insurance companies, media, research groups, etc. All questions should also be directed to our Records Custodian. The Records Custodian can be reached by calling our main telephone number (727-582-6800), and asking to speak with the Records Custodian.
