Directions:

- Print the form and sign and date it. Make sure you have read and understand the narrative.
- Send the completed form, along with the required doctor’s certification, to the address at the top of the form.

Mobile Home Park Exemption Request

Directions:
- Please complete sections 1 – 10 (Name of Mobile Home Park, Parcel Number, etc.) of the form.
- Print the form, have the authorized representative sign and date it, and print their name, title, and telephone number in the spaces provided.
- Send the completed form, along with the required collection contract, to the address on the form.
- If you have any questions, please contact the Lealman Coordinator at 727-464-7500.
  
  The form must be submitted by June 30th of the year prior to the exemption taking effect. Failure to submit the form on time can result in assessment for solid waste services.

Vacant Residential Unit Refund Request

Directions:
- Please complete sections 1 – 6 (Vacancy Period, Owner's Name,
- Telephone Number, Owner's Address, Tax Parcel ID Number, Parcel Address) of the form.
- Provide documentation for sections 7 - 9. You can verify payment of the assessment via the Tax Collector's database. Copies of the electric bills for the year can meet the requirements of sections 8 and 9.
- Sign the form and have it notarized. Send the completed form, along with the required documentation, to the address on the form.

- The form must be post marked by March 31st of the following year for which the refund is sought. Failure to submit the form on time is deemed a waiver of the right to a refund.
