Working with your insurance company to cover out-of-pocket ambulance expenses (co-payments or deductibles)

• Covers medically necessary Sunstar ambulance transportation to or from any medical facility in Pinellas County
  (Does not include Sunstar Mental Health Transport Van)
• Annual Fees are $46.82 per individual/$72.84 per family
  (Family Plan includes those related by blood, marriage or adoption, who reside in the same household.)
• No membership cards required
• Membership applications accepted beginning January 1, 2013 (early memberships accepted in November and December are not effective until January 1, 2013)

• Download an application and mail it in with your check or money order
