The Pinellas County Department of Health & Human Services is pleased to announce that it is entering into a cooperative agreement with the Pinellas County Health Department and the Community Health Centers of Pinellas County to provide comprehensive medical care to indigent and other residents of Pinellas County. Medical treatment will be offered for both preventive care and the treatment of illness or injury. Care will be provided in a comprehensive and professional manner utilizing the “medical home” concept that has been adopted by many communities across the United States in order to bring a new standard of health and wellness to the citizens of Pinellas County.
One of the primary goals of the program is to divert patients from the inappropriate and costly use of local emergency room facilities for their basic healthcare needs. Instead, patients will be encouraged to take responsibility for their own health status and will be urged to practice a healthy lifestyle including exercise, diet and regular medical checkups at their medical home.
The Department of Health & Human Services will begin to transition currently eligible patients from the existing healthcare program into the new program effective August 1, 2008. After August 1, new patients should visit one of the Health Department sites listed in order to register for medical and pharmacy assistance.
