arrow What is a Medical Home?

Trips to the emergency room for routine medical care are a thing of the past. That's because your Medical Home is your first stop for health care. A Medical Home is the place you can go for primary care and to help you receive other care offered in the Pinellas County Health Plan.

We will work with you to maintain a healthy lifestyle and coordinate health services. The professionals working in your Medical Home care about you and look forward to helping you live a healthier life. In addition, your Medical Home provides easy access to medical staff through same-day office appointments and some locations are open in the evening or on Saturdays.


arrow Do I qualify?

 To qualify for services, you must:

  • Be a U.S. citizen or legal resident
  • Be a Pinellas County resident
  • Be an adult between the ages of 18 and 64
  • Be uninsured and cannot pay for the medical care you need
  • Not be eligible for Medicaid, Medicare or other public assistance programs
  • Meet Federal Income Guidelines - see below

Income guidelines are based on the current Federal Poverty Guidelines for family size. These guidelines are updated each year. The figures shown are for net income after taxes are paid.

If you meet these requirements, you may apply for the Pinellas County Health Plan through a Medical Home. You can rest easy because the Medical Home staff offers professional services with no co-pays. Memberships generally last for one year.

 


arrow What does the program cover?
  • Doctor visits at your Medical Home
  • Referrals when necessary - only to approved providers
  • Mental health and substance abuse services
  • Wellness and prevention services such as
          - Annual physicals
          - Nutrition education and counseling
  • Cancer screening
  • Physical therapy
  • Pharmacy services
  • Referral for dental "relief of pain"
  • Laboratory and medical services such as:
          - Pap smears
          - PSA blood levels
          - Urine analysis including pregnancy test
          - General X-rays
          - Ultrasounds
          - MRIs
          - CT scans
          - Mammogram's

Case management services may be offered to help residents dealing with the challenges of:

  • Physical illness
  • Disability
  • Psychological concerns
  • Addictive behaviors

Please note that emergency room treatment and hospitalization are not covered. You are responsible for payment of these services.Be sure to ask for more information if you have questions about services you are eligible for.


arrow How do I sign up?

You must visit a Medical Home to sign up. Follow these four simple steps:

1. Look at the Medical Homes locations for the one nearest you.
   This is the enrollment site for the Pinellas County Health Plan.
2. Call to find out when it is open and get directions.
3. Get your paperwork ready. See list below
4. Go to the nearest Medical Home location to apply.

arrow What paperwork do I need?

Take a careful look at the documents you must bring on your first trip to a medical Home.

        1. Social Security card plus one other form of identification - Examples:

    • Birth certificate
    • Employment records
    • Florida identification
    • Mortgage or lease agreement
    • Student identification badge
    • United States passport
    • Valid driver’s license
    • Voter registration card
    • Work identification or badge

         2. If you do not have a Social Security card, bring proof of an application for a new card to prove you are a Pinellas County resident, bring one of these documents.

These documents must have a current address listed:

    • Bank statement
    • Employment record
    • Motor vehicle registration or title
    • School record
    • Pay check or pay stub
    • Rent receipt
    • Utility bill - Examples: landline telephone, electric, water, gas, cable

         3. Proof of legal citizenship or immigration if born outside the United States.

    • U. S. Naturalization papers
    • Resident card

         4. Proof of income for the past 60 days - Examples:

    • Award letters from the Social Security Administration or Veterans Administration
    • Child support/alimony
    • Pay stubs
    • Unemployment insurance benefits
    • Worker's Compensation

No income? Bring a support statement from someone who is helping you if you have no income. If you claim you have no income and no one is helping you financially, you will need to show proof of how you are paying your living expenses.

Please take a few minutes to review the Notice of Privacy Practices information. It explains how medical information may or may not be used and how you can get that medical information.

Please call (727) 464-8400 or (727) 582-7781 if you have questions that cannot be answered at your Medical Home. Be sure to leave your name, social security number, current address, phone number and the reason for your call.
