All medical care will be provided by a medical home to be selected by the eligible patient at one of the Pinellas County Health Department or Community Health Center of Pinellas, Inc. sites. Please see medical home flyers for additional information.

In order to be eligible, you must:

  • Have no insurance (including Medicare/Medicaid)
  • Be a Pinellas County resident
  • Be between the ages of 18-64
  • Be a U.S. citizen, legal permanent resident or sponsored alien
  • Bring pay stub for proof of income
  • Bring documentation to prove the above (examples: birth certificate, Social Security number, rental lease, or utility bills)

  • Referrals to specialists
  • Cancer screenings

Please note that Emergency Room treatment and inpatient hospitalization is not covered at the present time and will be the responsibility of the patient.

  • Effective October 1, 2008 health care to eligible patients of the Pinellas County Health & Human Services Department shall be provided by the Pinellas County Health Department (& their subcontractors) & the Community Health Centers of Pinellas, Inc.
