Appeal Form

The Appeal Form below should be completed and a check will need to be mailed to 12600 Belcher Road, Suite 102, Largo, Florida 33773 for $100 made payable to the PCCLB. We will attempt to schedule a hearing for your appeal approximately ten (10) working days after receipt by our office. If there is a particular date on which you are unavailable and you are willing to waive the 10-day hearing period, please so state on your Appeal Form. You, of course, will be notified of the hearing date, time, and place to present your position.

 

In the event you would wish to present such appeal without making a personal appearance, it may be presented on your behalf by your representative and by submission of such evidence as you desire.
